Download applicable claim form(s) and once completed and signed:

  • Mail to Manitoba Blue Cross at PO Box 1046 Stn Main, Winnipeg, Manitoba R3C 2X7
  • Drop it into the secure 24-hour claim box outside our building at 599 Empress Street, Winnipeg, Manitoba
  • Fax to 204.772.1231.
  • For life and disability claims ONLY, fax to 204.788.5591 or send by email.* (All other claims must be submitted through your mybluecross online account, in person, by mail or by the claims fax number noted above.)

For your claim to be processed, a copy of your receipt(s) and all other supporting documentation must be attached.

  • Extended health products (e.g. hearing aid, CPAP machine)
  • Extended health services (e.g. athletic therapy, massage therapy)

*Travel claims cannot be submitted in app. CanAssistance, our emergency travel assistance provider, processes travel claims on behalf of Manitoba Blue Cross. Visit How to submit a travel claim for more information.

Life and disability claims

Life and disability claims are not eligible for online submission.
