How to apply
If you are unable to work as a result of illness or injury, you may be eligible for short-term disability or long-term disability benefits through your employer’s group benefits plan.
A complete claim application requires all of the following:
1. An employer statement filled out by your employer
2. An employee statement filled out by you, the employee
3. An attending physician statement completed by your medical provider
Step 1: Inform your employer
If you need time off work, please speak with your employer directly. Your employer will submit an employer statement and other details required for your claim, including your employment and earnings information and job description. This information must be sent from your employer directly to Manitoba Blue Cross.
Step 2: Submit your employee statement
Please complete and submit the employee statement to provide us with essential information regarding your claim. It is important to complete each section to help us better understand your condition and review your claim.
Please ensure that your signed consent form is included as part of your employee statement.
You do not need to wait for your doctor or nurse practitioner to complete the attending physician statement before sending your employee statement to us.
Step 3: Have your medical provider submit an attending physician statement
The attending physician statement allows your physician, medical specialist or nurse practitioner to outline your current medical condition and how it impacts you.
Your medical provider will need to submit one of the following forms that best applies to your current situation:
Attending physician’s statement – Short-term disability
Attending physician’s statement – Long-term disability
Have your medical provider submit the completed form to us by faxing it to 204.788.5591. (This fax number is for life and disability claims ONLY.)
Step 4: Await a response
Once all three completed application forms have been received, we will review your claim. We may contact you directly to discuss your claim in more detail or request additional medical information on your behalf to help with our review.
You can expect a claim update or decision within five business days following the receipt of your complete short-term disability claim or within 10 business days following the receipt of your complete long-term disability claim.
Once a decision is made, we will send you a copy of our decision in writing, including details about our assessment and your benefits.
If your claim is approved, your disability case manager will contact you to discuss your recovery and return-to-work planning and answer any questions you may have.
